PROCESSING PASSPORT APPLICATION
(Basic Requirements)
FIRST TIME APPLICANT (Natural Born Filipinos)
- Personal appearance of applicant
- NSO Authenticated Birth Certificate in Security paper or Regional NSO authenticated BX issued by the Local Civil Registrar.
- Personal Identification (ID) with photograph of the applicant
- Three copies 4.5 cm x 3.5 cm new photos (colored with plain white background). Photo should show applicant in decent attire
- Submit either Baptismal Certificate/NBI Clearance/Voters Registration Record (If late registered just submit 2 documents)
IF NO BIRTH RECORD
- Born after 1950 require applicant to submit
LATE REGISTERED BIRTH CERTIFICATE authenticated by NSO
- Born on or before 1950, submit
Certificate of non-availability of birth record from NSO
Joint birth affidavit executed by two (2) disinterested persons
Any public documents indicating date, place of birth & citizenship
FOR MARRIED WOMEN
- NSO Marriage Contract in Security paper or Regional NSO
FOR MINORS (Below 18 years old)
- Personal appearance of applicant is required except those 8 years old
- Personal appearance of parent (s) of the legitimate minor childPassport of traveling companion
MINOR TRAVELING W/O THE PARENT (S)
- DSWD Clearance
- Affidavit of Support/Consent stating the name of travel companion
IF BOTH PARENTS ARE ABROAD
- Affidavit of Support & consent signed by parents
- Special Power of Attorney for the person authorized to apply for the minor
- Both documents should be authenticated by the nearest Philippine foreign service post
MINOR TRAVELING WITH ONE PARENT
- Parental travel permit from DSWD
FOR MINORS, 8 YEARS OLD & BELOW TRAVELING W/O THE MOTHER
- DSWD Clearance
- Affidavit of consent of mother
ILLIEGITIMATE MINOR
- NSO Authenticated BC SECPA or issued by LCR
- Personal Appearance and Affidavit of consent of the Mother
- DSWD clearance if minor is traveling without the mother
- In case the mother is dead or cannot be located, require Court Order on who shall exercise parental authority over the minor in accordance with Art. 216 of the Family Code
MINOR WHO’S MOTHER IS LIKEWISE A MINOR
- NSO Authenticated BC SECPA or issued by LCR
- Appearance of the mother
- Affidavit of support & consent of maternal grandparents
- Passport of traveling companion
- DSWD clearance if minor is traveling without the mother or the maternal grandparents
ADOPTED MINOR CHILDREN
- Authenticated copy of the original and amended birth certificate
- Authenticated copy and photocopy of the disparities portion of the Court Order on adoption
- Personal appearance of Adoptive parents
- Passport of the adoptive parents
- If minor is traveling w/o the adoptive parents, require DSWD clearance, Affidavit of Support & consent of parents, SPA for the person applying for the minor and the passport of traveling companion
ADOPTED MINORS UNDER INTER-COUNTRY ADOPTION
- Birth certificate or certificate of founding (SECPA)
- DSWD travel clearance
- Declaration of abandonment/deed of voluntary commitment
- Clearance for Inter –country adoption
- Child study report
- Placement authority
- Certificate/letter request for issuance of passport by ICAB
- Photocopy of passport of adopting foreign parents
ORPHANED MINORS
- NSO Authenticated birth certificate of minor
- NSO Authenticated death certificate of both parents
- Court order rewarding guardianship of the orphaned minor
- Passport of traveling companion
LEGITIMATED CHILDREN DUE TO SUBSEQUENT MARRIAGE OF PARENTS
- Authenticated Birth Certificate with annotation of the amended surname of the child
- Marriage Contract of the Child’s parents
MUSLIM – FILIPINO Applicants
- If birth was registered, follow requirements for new passport applicant
- If birth was not registered, submit CTC of late registered BC from NSO
- Voter’s Affidavit, NBI Clearance or other supporting documents indicating date and place of birth and citizenship
- Certification from the office on Muslin affairs for Muslim converts
- Court Order (Shari’ah Court) to change name
ACQUIRED PHILIPPINE CITIZENSHIP
BY NATURALIZATION (through judicial proceedings or Presidential Decrees)
- NSO authenticated BC of the applicant if born in the Philippines
- Bureau of Immigration ID of applicant
- Certificate of naturalization
- Oath of Allegiance
DERIVATIVE NATURALIZATION (wives and children of naturalized Filipinos)
- NSO authenticated BC of the applicant
- Applicant’s Bureau of Immigration ID

- Father’s Bureau of Immigration ID & certification of naturalization
ELECTION OF PHILIPPINE CITIZENSHIP
- NSO authenticated BC of applicant
- Bureau of Immigration ID duly-affirmed by the DOJ
- Oath of allegiance duly registered with LCR
- Affidavit of election of citizenship duly registered with LCR
BY MARRIAGE (alien woman married to a Filipino husband)
- Authenticated BC of applicant if born in the Philippines
- Authenticated marriage certificate of the applicant
- Bureau of Immigration ID duly affirmed by DOJ
- DOJ 2nd endorsement letter affirming Philippine citizenship by marriage
BY REPATRIATION UNDER P.D 725 (for women who lost Phil. Citizenship by marriage to aliens before 17Jan 1973)
- Authenticated birth certificate of the applicant
- Authenticated marriage contract of applicant
- Oath of allegiance registered with LCR
- Affidavit of repatriation
- Bureau of immigration ID duly affirmed by DOJ
BY REPATRIATION UNDER C.A NO. 63 (for widows of alien spouses)
- Authenticated birth certificate of applicant

II. RENEWAL OF PASSPORT
PERSONAL APPEARANCE NOT REQUIRED, EXCEPT UNDER
THE FOLLOWING CIRCUMSTANCES
- Passport has no BI stamp of latest arrival and departure
- Discrepancy appears in the passport data page
- Passport for renewal is declared lost
- Passport has lapsed for 10 years from date of expiration
- Passport holder’s name is included in LOL
REQUIREMENTS FOR RENEWAL OF PASSPORT
- Personal appearance of applicant
- Three copies 4.5cm x 3.5 new photos (colored with plain white background). Photo should show applicant in decent attire with collar
- Submit old passport and photocopies data page including the pages showing the latest departure and arrival stamps of the Bureau of Immigration
- If passport for renewal has a brown colored cover, submit any document showing the holder’s complete middle name
III. AMENDMENT FOR PASSPORTS
- Submit the original and photocopy of passport for amendment
- Authenticated marriage contract of the applicant (for amendment to married name)
- Authenticated death certificate of husband (to use maiden name again)

REPLACEMENT FOR LOST PASSPORT
- If already expired, submit notarized Affidavit of Loss w/ no penalty
- If still valid, submit Police Report and notarized Affidavit of Loss. A 15-day clearing period is required prior to processing the application for a new passport and the penalty will be imposed

- Birth certificate (BC) in Security Paper and supporting documents
